We are open to serve you, answer questions or schedule an appointment during the following hours:
Monday – Friday: 8 a.m. – 5 p.m.
*We are closed on New Year’s Day, Memorial Day, the Fourth of July, Labor Day, Thanksgiving Day, the day after Thanksgiving, and Christmas Day.
If you have a medical emergency, go to the nearest emergency room or call 911. If you wish to contact the physician on call, please call the Montgomery County Medical Society at: 937-463-1649.
New patients are seen by physician referral or self-referral, if allowed by your health insurance plan. It is important to bring all pertinent medical records to your appointment. If you are unable to keep your appointment, please call the office 24 hours in advance to cancel or reschedule your appointment. This will allow us to schedule the time for another patient.
Transfer of Medical Care
Request for transfer of medical records to another physician outside the practice requires a patient signed, medical release form. Please allow 48 hours for records to be copied.
Patients may be responsible for obtaining a referral from their primary care physician prior to their appointment if required by their insurance plan. Many insurance plans also require authorization from your primary care physician and/or precertification for certain tests and procedures (DXA Scans, MRIs, etc.). We depend on your knowledge of your plan’s benefits.
We recommend that you call your pharmacy for prescription refills before you are completely out of your medication, especially if a weekend or holiday is approaching.
Please allow 48 business hours for your prescription refill to be approved.
Refills are generally not processed over weekends.
