New patients are seen by physician referral or self-referral, if allowed by your health insurance plan. It is important to bring all pertinent medical records to your appointment.

If you are unable to keep your appointment, cancellations must be received one (1) working day in advance of your appointment. This will allow us to schedule the appointment time for another patient. Failure to notify us could result in a $25 "No Show" charge to your patient account. Your health insurance company will not reimburse you for a "No Show" fee.
